據統計卵巢癌佔臺灣地區婦女生殖癌症第二位,在子宮頸癌之後;而在已開發國家中,卵巢癌是女性常見癌症症中的第六位,也佔婦女因癌症造成死亡原因的第五位。有越來越多的證據顯示正常卵巢上皮細胞(OSE)及卵巢上皮癌(OEC)細胞所處的荷爾蒙環境,與卵巢癌發生及病程的進展有密切的關係。在生殖內分泌中性腺刺激素(Gonadotropins)及性腺刺激素釋放激素(GnRHs) 一般認為是卵巢癌發展的調節因素之一。然而,性腺刺激素及性腺刺激素釋放激素對卵巢上皮細胞及卵巢上皮細胞癌所造成影響的分子機轉尚未被清楚的瞭解。因此,本論文欲探討的是性腺刺激素對卵巢上皮細胞增殖的影響,及其可能的調控機轉;以及性腺刺激素釋

放激素對卵巢上皮細胞癌轉移能力的影響,及其對蛋白質水解(proteolysis)作用的影響。以作為進一步探討中醫藥在分子生物調控、卵巢腫瘤形成與轉移能力上所能扮演的角色的基礎。先前的研究顯示,性腺刺激素可以促進永生性卵巢上皮細胞(IOSE)的增殖。而此一作用乃透過活化ERK1/2 及 PI3K 訊息傳遞路徑,促進表皮增長因子接受器(EGFR)的表現量而達到。在這次研究中,我們主要探討cAMP及其特定的結合蛋白,即被cAMP活化的交換蛋白(Epac),是否在性腺刺激素調控EGFR表現中扮演主要角色。實驗結果顯示性腺刺激素可以增加IOSE和卵巢顆粒細胞(granulosa cells)內cAMP的

量。而這增量可被adenylyl cyclase (AC)的拮抗劑SQ22536所減弱。ERK1/2 和Akt路徑的活化作用以及EGFR表現量的升高,可被cAMP的類似物8-bromo-cAMP及AC的活化劑forskolin所刺激。相似的情行也可被另一個cAMP 類似物8-(4-chlorophenylthio)-20-O-methyladenosine-30,50-cyclic monophosphate (8-CPT-2ME-cAMP)所激活。而此一類似物不會活化PKA,只會活化Epac。而且,性腺刺激素所導致的EGFR增加和ERK1/2及Akt的活化作用也會因細胞缺乏Epac而阻斷。這

些結果表明性腺刺激素乃透過AC/cAMP/Epac此一訊息傳遞路徑,再活化ERK1/2和Akt路徑而增加EGFR的量。在轉移的實驗中,我們利用兩組卵巢上皮癌細胞株OVCAR-3(低侵犯性卵巢癌細胞株)及SKOV-3(高侵犯性卵巢癌細胞株),探討性腺刺激素釋放激素(GnRH - I 及GnRH-II)及其接受器對癌細胞侵犯性的影響。實驗結果顯示:GnRH-I和GnRH-II可以促進OVCAR-3細胞株的侵犯性,對SKOV-3細胞株則可減少其侵犯性。當把小干涉的核糖核酸(small interfering RNA)轉植至細胞株中,以阻斷GnRH接受器的基因表達,則可以消除GnRH-I及GnTH-I

I的此項作用。這顯示GnRH-I及GnRH-II可能藉由同一種接受器對細胞產生作用。用GnRH-I及GnRH-II處理SKOV-3細胞株,可以降低MMP2及增加其分解酶(TIMP2)的分泌。另外,我們發現GnRH-I和GnRH-II 也干涉了PI3K/AKT路徑的活化,而此一路徑過去的研究表明可以促進蛋白質水解及促進卵巢癌細胞的侵犯性。所以據此我們推測GnRH-I和GnRH-II對卵巢上皮癌細胞的侵犯性與細胞外基質的水解可扮演重要的角色。從以上的研究,顯示性腺刺激素及性腺刺激素釋放激素在正常與癌化的卵巢上皮細胞的增殖與轉移能力可扮演重要的調節角色。而這樣的經驗,也鼓舞我們繼續探討中醫藥作用的努
