探討黃蜂與火蟻叮咬傷造成之嚴重過敏反應病人臨床表現與處置及蜂膠萃取物抑制主動脈瘤生成研究

昆蟲是是地球上種類和數量最多的動物，因此昆蟲與人類必定會產生相互的利害關係。昆蟲造成人類傷害中，膜翅目叮咬受傷事件占有巨大份額，其中又以黃蜂類(Vespidae)、火蟻類(Formicidae)螫傷事件為多且較嚴重，因此本論文的主軸將以急診的角度，探討膜翅目昆蟲對人類所造成嚴重過敏反應 (anaphylaxis)與其他身體健康危害的問題，以及蜂膠成份中咖啡酸苯乙酯(CAPE)對於主動脈血管瘤生成上的影響。第一章是研究膜翅目昆蟲叮咬傷，其中以黃蜂類及火蟻類叮咬傷為急診兩大因動物致傷的來診原因，黃蜂和火蟻都是屬於膜翅目(Hymenoptera)昆蟲，所造成對人體的傷害和表現，十分類似，也時常造成
嚴重的過敏反應，火蟻叮咬傷患者出現過敏反應發生率更高，並且比黃蜂蜇傷患者在急診滯留時間更長。但黃蜂叮咬患者有更高的併發症和更高的入院率，尤其是更容易出現嚴重過敏反應。實驗室數據中，以肌酸激酶(creatine kinase)，對於急診評估黃蜂蜇傷患者的嚴重性有價值。黃蜂和火蟻叮咬傷發生率都與溫度呈正相關，並且在週末的發生比率也較高。第二章是研究急診嚴重過敏反應(anaphylaxis)之病人，在臨床治療上之各項評估。嚴重過敏反應(anaphylaxis)是可能致死的疾病，幾乎所有指引都將腎上腺素(epinephrine) 放在第一線使用的藥物，但是研究中發現近幾年來腎上腺素(epinephri
ne)在急診使用率卻仍然偏低，而病人的預後卻沒有因此不佳，歸究原因可能是急診治療包括給水、平躺、生命徵象維持，以及抗組織胺和類固醇使用，可能彌補了腎上腺素使用不足所帶來的不良影響，另外急診醫師也能選擇性地將腎上腺素用在有較嚴重過敏反應的病人上，例如:由救護車送診之病人、出現嚴重呼吸道症狀、低血壓及意識改變病人。第三章是研究探討蜂膠中咖啡酸苯乙酯 (caffeic acid phenethyl ester, CAPE)成份是否有抑制腹主動脈瘤生成的作用。在動物實驗方面，利用Ang-II 誘導的 AAA老鼠模型，CAPE能夠有減少主動脈直徑的擴大，另外透過先導性細胞實驗，發現CAPE可能通過抑制炎
症信號通路，例如p65激酶來抑制內皮層功能障礙。該研究的結果証明 CAPE具有抑制主動脈瘤成生的潛力。
臺灣市售肉桂藥材之鑑定及品質評估

桂為樟科Cinnamomum屬乾燥樹皮。肉桂含有高含量精油，主要成分為肉桂醛，具有甜味和特殊香氣之化學成分。肉桂自古以來廣泛應用於中藥、食品、化妝品和化工等行業。如今，在各國家藥典中，肉桂的三個主要品種包括Cinnamomum verum（稱為錫蘭肉桂）、Cinnamomum cassia（常稱為肉桂）和 Cinnamomum loureiroi（稱為西貢肉桂）。另外兩個品種Cinnamomum burmanni（陰香，假肉桂）之樹皮和Cinnamomum osmophloeum (簡稱土肉桂)之葉子也含有肉桂醛(cinnamaldehyde)。肉桂品種來源分歧，而且不同品種肉桂化學成分不同
，各國藥典對肉桂化學成分規範也有差異。因臺灣市場上肉桂品種不確定，進而影響肉桂品質及安全性。因此，本研究目的是辨別不同品種之肉桂以及評估臺灣市場上肉桂之品質。本研究收集來自越南中部和北越的兩批肉桂樣品（Cin-C 和 Cin-N）由批發商提供、十四批肉桂樣品（Cin-1 至 Cin-14）和十二批桂枝樣品（CR-1 至 CR-12）來自臺灣不同地區之市售品。進行顯微鏡鑑定、薄層層析（TLC）、高效液相層析（HPLC）、化學規格（總灰分、酸不溶性灰分、水抽提物、稀乙醇抽提物取物、乾燥減重）、二氧化硫測定及揮發油測定，用於鑑別和品質評估市售肉桂和桂枝。經實驗結果發現，顯微鏡鑑定，十六批肉桂樣品和十
二批桂枝樣品被鑑定為Cinnamomum cassia。通過TLC與HPLC方法可以測到五個標準品包括肉桂醛 (cinnamaldehyde)、肉桂酸 (cinnamic acid) 、香豆素(coumarin)、鄰甲氧基肉桂醛 (o-methoxy cinnamaldehyde) 和丁香酚(eugenol)。二十八 批肉桂和桂枝樣品皆含有肉桂醛 (cinnamaldehyde)、肉桂酸 (cinnamic acid) 、香豆素 (coumarin)和鄰甲氧基肉桂醛 (o-methoxy cinnamaldehyde) ；但皆無檢測到丁香酚 (eugenol)。因此，我們確認28批樣品都是Ci
nnamomum cassia，其中含有高含量的肉桂醛 (cinnamaldehyde) 和香豆素 (coumarin)。十六批肉桂樣品中的肉桂醛(cinnamaldehyde) 含量符合臺灣中藥典第四版之規範，但四批桂枝樣品不符合規範。桂枝中香豆素含量與鄰甲氧基肉桂醛呈負線性關係。同時，我們對越南的兩批肉桂樣品和臺灣市場上的十四批肉桂樣品進行評估和比較，我們確定肉桂外觀（厚度、顏色、味道）與其化學成分之間的關係。為了擴展本研究，在臺灣不同地區採集Cinnamomum cassia、Cinnamomum verum、Cinnamomum burmanni、Cinnamomum osmophlo
eum 四種之樹枝和葉子。通過薄層層析和高效液相分析發現四種樣品之化學成分有區別，其中土肉桂的肉桂醛 (cinnamaldedyde) 含量比其他品種高。也發現鄰甲氧基肉桂醛(o-methoxy cinnamaldehyde) 是Cinnamomum cassia的特殊之指標成分。二十八批肉桂和桂枝樣品的總灰分、酸不溶性灰分、水抽提物、稀乙醇抽提物、乾燥減重含量與各國藥典規範比較。於 香港中藥材標準2018和 中華人民共和國藥典2020 許多樣品不合格。所有 十六批 肉桂樣品均於 臺灣中藥典第四版揮發油含量規範合格。五批肉桂樣品和五批桂枝樣品皆未檢出二氧化硫，表示肉桂和桂枝未經硫磺熏蒸。本研究
