重症病人主要照顧者需求及社會支持之探討

研究背景：重症病人病情多屬於危及生命而需入住加護病房治療，且加護病房是不對外開放之病人，故被限制在外的家屬因其所愛的家人宣告病危而感受到沈重壓力，家屬在短時間面對壓力之負荷，若無法妥善處理壓力源，逐漸累積負面能量，就有可能造成疾病產生，評估家屬需求並透過社會支持，適當關心病人家屬其需求、給予支持可減輕壓力，協助家屬降低所受的衝擊，提高加護病房照護品質。研究目的：重症病人家屬需求與社會支持之相關性探討。研究方法：採橫斷式研究，以方便取樣方式，收集嘉南地區一所區域教學醫院，內、外科成人加護病房的家屬。研究工具採用結構式問卷，內容包括：｢基本人口學資料｣、｢重症需求重要性量表｣、｢社會支持量表｣等

三部份，採用SPSS 20.0軟體進行資料分析與建檔。統計方法包括：描述性統計、t檢定、ANOVA、皮爾森積差相關性分析、階層式逐步迴歸統計分析等。研究結果：本研究共收250位為研究對象，其中男性104人（佔41.6%），女性146人（佔58.4%），平均年齡47.91歲（標準差12.76歲）。研究結果顯示：重症病人家屬（1）需求重要性分析中，家屬之「保證」需求重要性得分較高，前三項為｢我能知道會影響病人病況進展的特殊事項，例如：新的感染、器官衰竭等｣、｢我能感受到醫護人員關心病人｣、｢醫護人員能夠使用我能了解的字句解釋病人的情況｣及｢能確保病人的到最好的照顧｣；（2）在整體社會支持總分中，以

「情緒性」的支持感受度越高，再依序為：評價性支持、訊息性支持、實質性支持，家人親友及醫療專業人員的支持均以｢情緒性的支持｣獲得最多；（3）重症病人家屬需求重要性與社會支持有顯著的相關性，顯示呈正相關，家屬需求重要性愈高則社會支持感受愈大；（4）重症病人家屬基本資料對需求重要性總分之迴歸分析，發現重症病人家屬的性別、病患年齡、病患入住加護病房時間、家人親友評價性支持對需求重要性有預測力。結 論：2020年嚴重特殊傳染性肺炎（COVID-19）疫情全球性爆發大流行，配合防疫政策，加護病房會客時間縮減及限制家屬探訪人數，並取消家屬休息室，研究問卷資料收集不完整，較為可惜，期待疫情解封訪視時間

恢復並開放家屬休息室使用，以利探討家屬需求與社會支持之間的相關性。
