以行動通訊軟體介入促進中老年糖尿病人血糖管理與降低負向情緒之成效探討

糖尿病的盛行率逐年上升，而血糖的自我管理與心理負擔是糖尿病人所需面對的主要問題。越來越多研究使用網路或是行動上網裝置來傳送衛教訊息、服藥提醒來幫助糖尿病人的自我管理。中老年人對於科技使用有較大的阻礙，也較少研究將科技介入運用在慢性病的中老年人身上。因此，本研究藉由行動通訊軟體”LINE”的介入，發送糖尿病衛教及相關的提醒訊息，協助目前已罹患糖尿病之中老年人提升血糖管理及降低負向情緒。本研究採類實驗設計，實驗組為2014年11月-2015年2月於家醫科及內分泌科符合收案條件之門診病人共49位，而2012年9月-11月於家醫科收案之門診病人，分成電話追蹤(N=91)與一般門診衛教(N=91)兩組
為本研究之對照組。研究結果指出，在糖尿病中老年人中，最多人發問的是飲食問題(26.7%)及藥物諮詢(26.7%)；其次，相對於其他兩組，在降低糖化血色素方面，藉由行動通訊軟體介入糖尿病衛教相較於電話追蹤組與一般門診衛教組有較顯著的進步(P=0.02)；而在降低負向情緒方面，行動通訊軟體組的糖尿病困擾分數有顯著降低(P=0.01)，與其他兩組比較，行動通訊軟體組也有較顯著的降低糖尿病困擾(P=0.02)，除此之外，行動通訊軟體的介入在中老年糖尿病人身上有其可行性，也能讓糖尿病中老年人感覺到知識、態度、行為的改變。
以社交網路平台支援糖尿病自我管理之可行性研究

背景：有鑑於糖尿病的自我管理主要於院外執行，一旦有任何問題，端賴專業醫療人員提供醫療照護，恐將不及逐年快速攀升的罹病人口所衍伸之醫療照護需求，因而陸續有研究證據顯示來自家人、朋友、同儕等提供的非正規社會支持同樣能為糖尿病自我管理帶來正面影響。隨著社交網路平台的盛行，不僅提供一個新管道聯繫人際間的關係，更藉資訊科技的利基加速溝通的效率，並期待此資訊科技工具能為支援糖尿病醫療照護服務與社會支持帶來益處，不過現有研究成果卻很有限。目的：(1)了解糖尿病患者目前執行自我管理時，獲得那些專業與非專業社會支持之現況。(2)了解可以透過社交網路平台滿足那些糖尿病患者所需的專業或非專業社會支持需求及其使用意
願。(3)探索社交網路平台使用意願的影響因素。方法：使用研究者自行開發並經專家效度之問卷，問卷內容包含基本人口學資料、糖尿病罹病情形、自我管理現況、科技使用經驗、以及社交網路平台的使用需求和意願，均採自述回答。研究對象皆為糖尿病患者，並至醫療院所採立意和滾雪球取樣方式招募受訪者。結果：共35筆有效分析資料（平均年齡為59.83歲、標準差為12.08），病人的社交網路平台使用需求主要是獲得糖尿病飲食或運動相關的衛教資訊，其次為詢問有關控制糖尿病時所遇到的問題，提供支持的對象則希望是透過醫療人員和家人。使用意願在年紀較輕有顯著差異，不過年齡介於60-79歲又聽過或使用過社交網路平台的話，使用意願會
顯著提高。結論：社交網路平台的使用意願以整體的平均數3.41來看是偏向正面，使用意願較高的族群特性為年紀較輕、第一型糖尿病、罹病年數5年以內和疾病嚴重程度較高者。這些研究成果將可進一步了解那些特性的族群適合透過社交網路平台來支援糖尿病自我管理。
