目前可以得知鎳鈦旋轉器械在根管治療中比手動式不鏽鋼器械有著更好的柔韌性與切削效率,且能夠有效縮短手術時間及提高品質,儘管有以上諸多優勢,但仍然會有無預警斷裂的風險存在。在臨床上廠商可能會提供醫師使用器械之順序,而大多使用過多器械,形成不必要之程序,進而導致治療時間過長。有鑑於此,本研究將設計新型鎳鈦器械,預期可有效減少根管治療所需的器械數量(4支),且透過器械切削根管期間的扭矩值做為新型器械使用的安全指標。本研究首先根據臨床牙醫師建議,建立一套標準化的簡化直根管,並定義適合該根管形狀之理想切削範圍,以此理想根管切削範圍做為新型器械系統設計之依據,再依序建立不同彎曲角度之根管進行分析。設計的新

型器械先利用所建立之扭矩預測表進行扭矩值初步評估,再將其透過動態有限元素分析模擬其根管治療期間的切削行為,並觀測器械之扭矩值是否超過斷裂標準,若扭矩值過高則再次調整新型器械中的切削範圍分配量,而器械扭矩值之安全範圍將其設定為2 Ncm ~ 3 Ncm內。在考慮到本研究設計之新型鎳鈦旋轉器械中,第三支器械在根管修形會切削至最深處,且相較於其他器械有最小之尖端橫截面積,導致其最容易發生扭轉斷裂;因此,本研究將以新型器械系統中第三支器械之扭矩值作為評估之重點。另外,在設計新型鎳鈦旋轉器械應避免器械長時間進行全工作長度的切削,以防止器械承受過高的扭矩值。結果顯示,4支器械在切削直根管的扭矩值分別為2.

64 Ncm、2.97 Ncm、1.75 Ncm、1.72 Ncm,並成功設計一套可在安全扭矩值範圍內切削之新型鎳鈦旋轉器械系統。在與直根管相比下器械切削彎曲根管之扭矩值皆落在所設定的安全扭矩值範圍內。而觀測單一節點應力方面下,在彎曲角度越大時,應力則越大。本研究不僅評估器械之扭矩值也觀測其應力,發現當根管之彎曲角度越大,扭矩與應力就會跟著提升,因此,遇到越嚴重彎曲之根管,則在治療期間就需越注意其安全性。新型鎳鈦旋轉器械設計之方法為先使用尺寸較大的器械進行根管冠端之切削,再利用變錐度設計之器械進行後續切削以防止扭矩值迅速提高。而在不考慮因循環疲勞破壞的條件下,可透過簡化直根管來評估新型器械之設

計,並設定在使用4支器械之內完成根管修形,預期能在臨床上有效提升根管治療的效率。
