集群分析之飲食型態與睡眠品質之相關:輪班之影響

背景與重要性: 在臺灣,輪班工作者超過20%。輪班工作可能使晝夜節律紊亂,影響食物選擇及睡眠,導致不良的健康問題。睡眠品質與飲食的攝取亦有相關;文獻指出多吃魚及蔬菜,睡眠品質較佳;碳水化合物攝取越多,睡眠品質則有下降的趨勢。飲食型態可反映出個人較整體飲食的複雜性,是探討飲食與疾病關聯的一種選擇;其中,集群分析依飲食特性,將研究對象分組。針對輪班工作、以集群分析之飲食型態及睡眠品質三者之本土研究仍少。研究目的:探討輪班工作者集群分析之飲食型態與睡眠品質之相關。研究方法: 本論文為橫斷性研究,研究參與者為2018年4月至2020年3月期間至三軍總醫院內湖院區健康中心體檢者,納入條件為有工作且

年齡介於20-70歲者,輪班與非輪班工作者約各半,共600名,均簽署了知情同意書。資料蒐集包括人口學資料、健康行為、半定量食物頻率問卷(FFQ)、睡眠品質資料等資料。先將FFQ所收集參與者過去一個月的各項食物及頻率,換算為每日每仟卡攝取克數。續藉由因素分析中之主成分分析方法,將食物項目縮減維度,產生六個飲食型態。最後,以k平均數集群分析,將參與者按其飲食特性分組。自覺睡眠品質分為良好、普通、不佳三類。以多項式羅吉斯迴歸,探討集群分析之飲食型態及睡眠品質的關係。另外,使用原先睡眠品質單題加上另外四個睡眠相關題目,創建睡眠品質分數,以線性迴歸,探討集群分析之飲食型態與睡眠品質的關係。結果: 集群

分析,將參與者分為肉食飲食者(n=356)、蔬果飲食者(n=114)及零食飲料飲食者(n=130)。三組中,輪班工作者比例分別為45.8%、46.5%及53.8%。輪班工作者睡眠品質不佳之比例(17.8%)高於非輪班工作者(11.8%)。飲食型態、性別、疾病史、輪班與否、輪班類型皆與睡眠品質有關。在複迴歸模式中,與肉食飲食者相比,蔬果飲食者睡眠品質不佳的風險顯著較高(OR:1.93,95% CI:1.04-3.59)。各種分層分析中,女性(OR:2.06,95% CI:1.01-4.34);無運動習慣者(OR:6.23,95% CI:1.26-30.7);無疾病史者(OR:2.06,95% C

I:1.01-4.19),此現象依舊顯著相關。年齡大於31歲者(OR:2.11,95% CI:0.84-5.32),輪班工作者(OR:1.99,95% CI:0.87-4.62),無固定輪班工作者(OR:1.21,95% CI:0.35-4.17),雖未達統計顯著,但方向與原來分析一致。另以睡眠品質分數來看,控制共變項後,飲食型態與睡眠品質無顯著相關。結論: 三種不同飲食型態的人,零食飲料飲食者輪班比例最高;輪班工作者睡眠品質較非輪班工作者差。然而,比起肉食飲食者,蔬果飲食者睡眠品質較差,與文獻相悖。關鍵字:飲食型態、集群分析、因素分析、輪班工作、睡眠品質

台灣電子商務營業稅課徵之探討

近年來數位經濟快速發展,商業模式的變化也十分迅速,在如何使無論是境內或境外電子商務皆能在台灣發展的前提下,政府負起保護國家之稅收來源,因此政府課徵營業稅之問題,成現今各國政府重要之議題。OECD在2013年提出「稅基侵蝕與利潤移轉報告」 (Base Erosion and Profit Shifting Project, BEPS),該報告被視為各國在課徵稅務時遵循之規範。而在發展數位經濟下,報告中BEPS行動方案1為電子商務課稅時可參考之方向。我國在2005年制定網路交易課稅規範,到2017年修改營業稅法,雖然在這方面已經解決不少問題,如透過稅籍登記之辦法,可清楚知道電子商務收入之狀況等,

以便進行課稅,但由於商業模式的演變,如何合理的課稅以及取得企業營運金流資料成重要之課題。本研究將提出現行課徵電子商務營業稅時,可能發生之困難,如Booking.com,因交易對象及營業場所之定義不同而造成不同之租稅負擔;針對Uber個案在如今課稅之狀態,並探討2012至2016年時課徵營業稅之最高行政法院2018年度判字第199號判決案之問題,最後針對新興虛實融合模式之松鼠AI等個案,分析我國課徵營業稅之問題,並提出因應之辦法。本研究結果可發現仍然有境外電商尚未完成稅籍登記,此結果恐使我國營業稅稅源流失,除此之外在Uber判決案中處理之模式,雖然該案為本國在課稅面之楷模,但本案後其他大型電商卻

無採用相同標準審視,著實可惜。最後以中國課稅模式探討之松鼠AI可發現在扣抵時如何劃分合理的支出,以及若營運模式收入判斷是否列入課稅面。我國政府須重新規範扣抵之範圍,並將費用合理的劃分,讓我國產業能夠順利的發展。
