中文摘要研究背景及目的為了減少道德危害風險,我國全民健康保險有部分負擔之設計。為配合分級醫療政策之推動,在歷經多次部分負擔調整後,2017 年 4 月 15 日門診部分負擔的調整與過往不同,調漲未經轉診至醫學中心就醫者,並調降經轉診至區域醫院及醫學中心就醫者。過去有關部分負擔的研究大多在探討部分負擔對醫療利用及費用之影響,只有少數探討其對醫療服務有效性之影響。糖尿病是一種高發生率、高盛行率、多元照顧的慢性疾病,若能早期診斷並透過適當的糖尿病照護管理可控制血糖、可降低其併發症風險和延長壽命。本研究目的在探討 2017 年新制部分負擔政策對糖尿病患有效醫療服務的影響。研究設計本研究以Donabe
dian之SPO模式及Andersen的第三代健康服務利用行為模式作為理論架構,採取類實驗設計(quasi-experiment study)中的事前事後控制組比較研究法(pre-post with-without comparison)。使用全民健保資料庫的資料,以2017年4月15日部分負擔政策的介入作為切點,比較實施前後1年對有效醫療服務利用的變化,介入組為至醫學中心及區域醫院就診之保險對象;對照組為至地區醫院及基層診所就診之保險對象,以1:1傾向分數(propensity score)配對基線時病患特性之性別、年齡、疾病嚴重度、共病等四項,以減少兩組的選樣誤差。有效醫療服務係參考美國國
家品質確保委員會委員會(NCQA)發展的健康照護有效性指標的資料庫(HEDIS)中,以完整的糖尿病照護 (Comprehensive Diabetes Care,簡稱 CDC)指標進行評估。為配合全民健保論質之付之規定,以醫療過程指標為主,並計算糖化血色素、總膽固醇及低密度膽固醇、眼底檢查及微量白蛋白檢驗等檢驗以及其過程品質綜合指數。再以廣義估計方程式 (Generalized Estimating Equation, GEE)分析差異中的差異法(Difference in difference, DID)之政策效應。 研究結果研究結果發現,在控制其他影響因素後,進行GEE分析發現部分負擔對醫
療過程指標中的糖化血色素、總膽固醇及低密度膽固醇檢驗等項目無顯著影響;但眼底檢查及微量白蛋白檢驗兩項檢查在新制部分負擔之介入後有顯著減少之趨勢;糖尿病患之過程品質綜合指數則顯著增加。研究結論及建議本研究結果發現,部分負擔政策之實施對糖尿病患之常規執行的糖化血色素、總膽固醇檢驗以及低密度膽固醇檢驗無顯著影響;但對需轉診的眼底檢查及微量白蛋白檢驗則顯著減少。建議政府制定或調整部分負擔政策時,應考量慢性病患照護之需要,並定期監測慢性病患之有效醫療服務利用情形,以避免因政策而影響到特定族群使用有效醫療服務之權益。
