治療困難憂鬱症是嚴重的精神疾患,將會造成個人與社會的巨大後果,包括:自殺。西方研究顯示,低劑量ketamine靜脈滴注對於治療困難憂鬱症之個案,有顯著且快速的抗憂鬱效果。而此快速抗憂鬱效果,動物實驗顯示可能機轉為增進大腦神經滋養因子(BDNF)表現。但西方人種和台灣人之神經滋養因子的基因多型性表現差異甚大,前者以Val為多,後者則以Met為多。且至今,尚未有臨床實驗研究是否低劑量ketamine滴注對於治療困難憂鬱症之台灣個案有快速抗憂鬱之療效。本研究乃一隨機分配且對照組之臨床研究,共納入71位治療困難憂鬱症台灣個案,隨機分配三組治療,分別為:0.5mg/kg、0.2mg/kg之四十分鐘ke

tamine滴注,或生理食鹽水為對照組滴注。完整之精神症狀、認知功能、發炎因子、正子攝影、和功能性核磁共振攝影於滴注前和滴注後妥善評估並追蹤。研究結果顯示,單一施打之低劑量ketamine滴注,尤其是0.5mg/kg,對治療困難憂鬱症有快速且顯著之抗憂鬱效果,其治療反應率約五成,治療反應定義為百分之五十之憂鬱症狀下降。然而,本研究發現,大腦神經滋養因子之基因多型性並非ketamine滴注抗憂鬱療效之預測因子。認知功能分析顯示,單一劑量ketamine滴注,並不會傷害個案之認知功能,甚至在0.5mg/kg組可以輕微地增進認知功能。分析發炎因子發現,低劑量ketamine滴注可以快速調節發炎因子濃
