不同水溫下水中行走對過敏性氣喘大鼠之效用

背景:臺灣在2017年的氣喘盛行率占了近5%,顯示即使現在醫療進步,仍有許多人受氣喘所苦。氣喘,有時稱為哮喘或支氣管氣喘,是一種慢性呼吸道發炎的醫學狀況,病人可能包含以下臨床表徵:胸悶、咳嗽、呼吸短促、以及哮鳴聲,並且伴隨不定程度的氣流受限。現今的治療主要使用類固醇,雖然它已知有良好的抗發炎效果,其長期使用也會帶來其他併發症,因此非藥物介入就成了另一種選擇。本篇研究欲探討的非藥物介入為運動,特別是水中行走訓練,其對於呼吸道發炎情況帶來的緩解效果,也希望觀察不同水溫對於治療氣喘的影響,基於需深入了解背後的機制,本實驗會使用大鼠,並取出特定組織進行分析。方法:雄性大鼠(Sprague-Dawle

y [SD] rats)被隨機分組:控制組、氣喘組、類固醇治療組、29°C水中行走訓練組、36°C水中行走訓練組。類固醇選用dexamethasone,而水中行走訓練分別於29°C與36°C的水溫下進行。過程中會收集尖峰吐氣流速、潮氣容積、呼吸頻率、與每分鐘通氣量的數值,之後會取得血清和支氣管肺泡灌洗液,分析當中的介白素-4、介白素-5、介白素-10、與免疫球蛋白E,白血球數量以流式細胞儀計算,而肺臟組織則用於觀察局部病理狀況。結果:實驗中誘發氣喘後,大鼠最常出現哮鳴聲與咳嗽,而控制組無明顯症狀。根據體重變化,只有在類固醇治療組的大鼠體重會先明顯減少,而後維持一定數值至犧牲當天,其餘組別間無太

大差異。肺功能測試中,氣喘組於呼吸頻率有顯著較高的數值,也發現36°C水中行走訓練組在後面幾天的呼吸頻率開始下降。蛋白質分析顯示,類固醇治療組有較高的介白素-4,以及較低的介白素-5、介白素-10、與免疫球蛋白E,至於兩組水中行走訓練組與氣喘組相比則無明顯差異。白血球數量分析顯示,類固醇治療組有較少的淋巴球,而29°C水中行走訓練組有較少嗜酸性白血球的趨勢。組織病理分析顯示,類固醇治療組有較少的發炎狀況,而兩組水中行走訓練組與氣喘組相比則無明顯差異。結論:早期讓大鼠開始在36°C的水溫下進行行走訓練,可緩解後期誘發氣喘造成的呼吸頻率增加之狀況,但仍需更多分析與後續實驗證實此現象。
