摘要研究背景與研究目的:  跌倒是台灣老年人事故死亡的第二名,而跌倒後可能導致骨折、住院、失能和死亡,因此跌倒對於老年人而言,是一個會影響日常生活和健康的危害。老年人和年輕人相比,於執行雙重任務時較容易有步態不穩定的情形,進而造成跌倒的發生,因此了解雙重任務對長者步態的影響才能發展合適的復健和預防措施。日常生活中常見的雙重任務包含視覺、認知和動作等形式,而這三種形式的雙重任務皆會影響老年人的步態表現,然而目前較少文獻探討有無跌倒經驗的老年人在不同雙重任務情況下的步態表現有何差異,故本研究的目的為探討日常生活中不同形式的雙重任務(視覺、認知和動作)對有跌倒經驗老年人步態表現的影響。研究方法: 

 本研究為橫斷性研究,從台灣北部社區招募22位老年人,包含有跌倒經驗的老年人11名跟沒有跌倒經驗的老年人11名,評估的項目包含有認知功能、功能性活動能力和雙重任務(視覺、認知、動作)步態表現,接著使用獨立t檢定、單因子變異數分析和事後檢定檢驗組間和組內的差異。研究結果:  結果顯示兩組在執行單一任務行走時的步態表現,非跌倒組的步態週期時間顯著低於跌倒組(p=0.042);在跨越障礙物這項視覺雙重任務的情況下,非跌倒組的步態節奏顯著高於跌倒組(p=0.042);在所有認知雙重任務的情況下,兩組的步態參數皆未達顯著差異;在端裝水杯的托盤這項動作雙重任務的情況下,跌倒組的步態節奏(p=0.005)、

步態週期時間(p=0.007)顯著較非跌倒組的表現差。此外,非跌倒組在扣釦子時的步態週期時間(p=0.016)和步態週期時間變異度(p=0.021)顯著較單一任務行走時大。結論:  本研究發現動作雙重任務對有跌倒經驗老年人的步態參數表現影響最大,視覺雙重任務次之,認知雙重任務的影響最小,故在設計預防跌倒的訓練計畫時,可將此結果列入考慮,本研究的樣本較小,未來需要納入更多的個案以驗證此結果。
